For your first visit, please download the following forms, fill them out and bring them with you to your consultation appointment.

Patient Information Form
Medical History Form
Orthodontic Informed Consent Form

We are always excited about meeting new patients during their first visit and letting you get to know us!

Your first orthodontic appointment will consist of an examination and a discussion of potential treatment options with one of our doctors, x-rays, and meeting with our treatment coordinator to discuss types of braces and aligners and payment options.

X-rays will be made of your teeth and jaws so that our orthodontist can evaluate these while examining your teeth, occlusion, facial profile and jaw structures. This information provides our orthodontist with a complete understanding of your case and the necessary information to develop a treatment plan.

There is no charge for this initial consultation or for any x-rays taken at this appointment.

At the end of the exam, the orthodontist will:

  • Advise whether or not orthodontic treatment is recommended
  • Determine when orthodontic treatment should begin
  • Present the possible orthodontic treatment options available
  • Formulate a definitive treatment plan to achieve, as close as possible, your goals and expectations

The initial exam usually takes about an hour. Part of this time is spent filling out patient information forms. In order to save time, please complete the patient information forms which are available at the top of this web page. By completing these forms before you come in, we can start your first visit without delay.

After the consultation appointment, if you decide to get started with treatment, orthodontic records can be taken the same day. We offer a 50% discount on the price of records if you do choose to complete the records the same day as your consultation appointment.

The records consist of impressions for diagnostic models of your teeth and photographs of your face and teeth. Once the records have been made, you will be given an appointment to have the braces placed. It’s that simple!

Insurance

If you have orthodontic insurance, you need to contact your provider before the first visit to know how much your policy pays. The questions you need to ask your provider are:

  • Do I have orthodontic coverage?
  • What are the benefits of my orthodontic coverage?
  • Is my orthodontist a provider for this insurance? If not, will it cost more to use a non-provider orthodontist?
  • Does the insurance company pay automatically?

All questions regarding your insurance policy must be directed to your carrier. We accept many insurance providers, please call our office to find out which providers we accept. We file initial insurance claims the day braces, aligners or appliances are placed on the teeth. We will file any insurance claim for you provided you do have orthodontic coverage. Please bring your insurance card with you to your consultation appointment.
